To enter, click here and complete the official entry form including your name, address, and phone number between September 1, 2013 and September 30, 2013. On or about October 1, 2013, a random drawing from all eligible entries will be conducted to determine the winner. The winner must sign and execute an Affidavit of Eligibility and Release and a Form W–9 (Request for Taxpayer Identification Number and Certification) within 5 days of receipt in order to officially accept and receive the prize. The travel companion will also be required to sign a Release of Liability Affidavit. If the selected winner is ineligible, fails to execute and return the documents as required or is unable to travel on the dates specified, an alternate winner may or may not be selected.
One Grand Prize of a trip for two to Los Angeles, California to attend the Lionsgate movie premiere of "The Hunger Games: Catching Fire" in November 2013 will be awarded. The prize consists of coach airfare for 2, accommodations at a 4 star hotel for 2 nights, limousine service to and from the hotel to the movie event, a $500 HSN gift card for the winner and a basket of beauty including products selected by HSN host, Amy Morrison. The total retail value of the prize is approximately $2400.00.
